Catalogue and Web Page pricing may change without notice. Action Forms Inc. reserves the right to notify of incorrect pricing due to additional charges, price changes, general errors etc. prior to proceeding with process of the order(s).
All pricing is based on all parts of the form being identical. Please contact a Customer Service representative for additional pricing if there are any differences between the parts of your forms i.e.: part 3 has a blockout.
All pricing is for 1 standard ink colour, except where otherwise noted.
An artwork proof will be faxed or emailed for your approval. You are fully responsible for any errors that you fail to correct, this includes all spelling!
No shipping dates will be confirmed until all artwork has been approved. Standard order times are 2 weeks from date of approved artwork (except stock orders and economy cheques).
Stock items are shipped within 48 hours from receipt of order, unless backordered.
Billing terms are Net 30 Days from OUR shipping date payable cash, check, Visa or MasterCard.
All imprinted orders are subject to industry standard +/- 10% overrun. Please call with any questions.
Prices do NOT include applicable taxes or freight.
Prices DO include artwork, subject to our approval.
Artwork is free ONLY with an completed order. Canceling an order
will result in artwork charges ($65 per hour, minimum $35).
FORM / STATIONERY
ORDERS
It is necessary for you to inform us of any changes you have made to the layout of your software when order software compatible forms.
The paper sequence for "# of copies" will be as follows, unless otherwise noted:
1 copy - WhiteTo add a consecutive number to any forms an additional charge of $25 will apply. All numbering is in red unless otherwise noted. An additional $10 fee will apply for black numbering.
2 copies - White, Yellow
3 copies - White, Yellow, Pink
4 copies - White, Yellow, Pink, Goldenrod
If you choose to add your company logo, there is no charge
to do so, however the logo will appear in the same ink colour as the form.
Have your logo printed in a different colour, an additional fee will apply
based on colour choice ($65 for black and $95 for any additional colour).

LOGO
If you wish to add your company logo to any of the forms
there is NO additional charge, however we must be provided with a clear,
scannable copy of your logo. Please mail, courier or email your logo
(a faxed copy will not be accepted). If possible please supply us
with more than one sample i.e.: a sample business card, letterhead
and envelope, so that we may chose the best sample for scanning.
(Please call prior to emailing to discuss specifications).
The final quality of your logo depends on the quality
of artwork you provide us with, we do not touch up artwork.
INK
Our standard inks are as follows:
Dark Blue (Reflex Blue)
Light Blue (Process Blue)
Green (PMS 347)
Burgundy (PMS 221)
Red (PMS 032)
Tan (PMS 154)
BlackFor any other ink, an additional $65 mix charge will apply per order.
Please note: All inks are not available for all products.
